Rate the level of workplace organisation, if 1 equals disorganised, cluttered and untidy and 5 were to equal everything has a home, good housekeeping and logical order to storage of tools, materials and equipment etc.

Rate the layout of your workplace, if 1 were to equal the work area being all over the place with lots of movement and transportation, and 5 were to equal there being very well set out process steps with little transportation waste and wasted floor space.

Rate the visual management of your workplace, e.g. if 1 equals there being very few visual signs, you have to ask to find out what work is in progress and how the depot is performing - and 5 equals that it's very clear what is being done and when things are being worked on, levels of stock & materials are clearly identified and problems are quickly spotted and corrected with key performance measures being used and kept up to date.

Rate the process of disposing of your wastes. If 1 equals wastes are not tracked, with obvious waste of all types all over the facility; and 5 equals wastes are being regularly tracked and are significantly reduced using appropriate improvement activities.

Rate your standard operating procedures. If 1 equalled a lot of variance between jobs, people do what they like most of the time; and 5 equalled work procedures were consistent, taught to and understood by the team. All processes have been optimised, balanced and validated.

Rate the pace of work in your organisation. If 1 equalled work levels are uneven, and 5 equalled that the rate of work is governed by the customer takt time and the schedules are balanced

Rate the quality of team working within your organisation. If 1 equals people work as individuals, not teams, and the ‘them and us’ atmosphere is rife; and 5 equals teams are empowered and self managed and continuously improve. Shop floor involved in and informed of improvements.

Rate how multi-skilled your employees are. If 1 equals people tend to work on one activity, job or machine; and 5 equals people are flexible, multi-skilled and can move around the organisation to help in teams from other areas.

Rate your own continuous improvement activities. If 1 equals there being low involvement or interest in continuous improvement, it's viewed with suspicion and cynicism within the company; then 5 equals there being a high level of staff involvement, reward systems in place. Real benefits being achieved for the teams and organisation.

Rate your organisations internal reaction to errors. If 1 equals workers aren’t encouraged to identify problems because of production pressures, errors can go undetected for some time; then 5 equals everyone can and does stop the process to rectify problems. Problems are resolved quickly and efficiently.

Rate your organisations ability to process root cause analysis. If 1 equals symptoms are tackled but the problem re-occurs – no root cause established and no methods for identifying root cause; then 5 equals everyone involved, the problems are permanently fixed.

Rate the level of support your senior management provides. If 1 equals there is no senior management support – it's viewed with suspicion and cynicism; and 5 equals there is full support from the top management. Continuous Improvement built into the business plan.

Rate the level of efficiency in your internal processes. If 1 equals processes aren’t logged or recorded and the value streams aren’t identified; and 5 equals there is a fully recorded process maps for the business, all the team actively involved in understanding the business process.

Rate the level of variation in processes within your organisation. If 1 equals variation in processes aren’t known or recorded, no systems in place to measure variation; and 5 equals variation is known, measured and recorded. All common cause variations are measured and reduced. Very small levels of process variation and good six-sigma control.

Rate the level of process control within your organisation. If 1 equals there is no process control in-place, no methods of recording process control, staff not aware of the benefits; and 5 equals statistical process control practices are in place, staff fully conversant with SPC practices and benefits.

Rate the process risk within your organisation. If 1 equals there is no practices in place to identify risks in the process; and 5 equals there are failure mode effects analysis being used by the business.

Rate the data collection within your organisation. If 1 equals data has not been checked for accuracy, irregular collection of key data, no consideration for sampling size; and 5 equals data is accurate, it has valid sample sizes, the measurement system has been evaluated and data collection is processes are in place.

Rate the data analysis within your organisation. If 1 equals data is collected but not analysed, stored away in files and not reviewed; and 5 equals the data is accurate, it is regularly analysed and root causes of problems are investigated and eliminated.

Rate the improvement implementation within your organisation. If 1 equals there is an ad hoc approach to implementing improvement suggestions based on analysed data; and 5 equals the team is fully capable of using design of experiments to minimise risk and increase the likelihood of successful.

Rate the improvement definition within your organisation. If 1 equals improvement methodology not defined, no clear or standardised approach to defining the problem or recording the benefits to the business; and 5 equals there is a fully defined project definition including the voice of the customer, stakeholder and buy-in. Benefits identified and the opportunities logged.
